I must admit I used the sheet music book to get the basic notes for the melody, but I came up with the whole arrangement, background chords and stuff myself though. I can never rely solely on the official sheet music books, since they aren't always accurate to the actual song on the soundtrack, so I always listen to the original song as I'm doing my midis to get the right tones, flow and instruments (or at least as close as possible).

Nope the program I use requires you to add each and every single note in by hand and that's what I did throughout the entire song.

But were the program such that you could play it and it would record it I would have been able to do that as well.

I'm using a program called NoteWorthy Composer. You have to be able to know and read sheet music to work it (and not just know the notes but know everything around it, like keys, chords etc). There are probably tons better programs out there but I like it because it's what I'm used to (after all I've used it for over 5 years now!)
