Actually, permantently it was removed from the circles of the earth after the Great Armament. When Ar-Pharazon built a fleet, and landed with the army of the numenoreans on the undying lands, intending to wrest power from the Valar. At that point the realm of valinor was withdrawn from the circles of the world.
And Fingolfin was slain in one-on-one combat with Morgoth (if that is what you meant)

Lol, AWD. Maybe that would work.
Lake Linaewen was in Nevrast. Translated it means "Lake of Birds" or something like that.

Nope. It is a given fact--at one point, Tolkien refers to one of them as 'the mightiest of the Edain.' I'd give you a source, but that kind of defeats of the purpose of asking the question in the first place.
